Unofficial Transcripts:
Unofficial transcripts are PDF or printed copies provided for personal use. They are not certified or sealed and are available only via email or in-person pickup at the Main Office.
Official Transcripts:
Official transcripts are certified documents sent directly to institutions or organizations in sealed envelopes to ensure authenticity.
Current Students: Current students or their parents/guardians can request transcripts by visiting the Main Office during business hours or by completing the transcript request form.
Seniors Applying to College: Your transcripts will be reviewed and sent directly to your colleges in which you are applying by your college counselor through Maia. You do not need to submit a separate request—just stay in close communication with your college counselor throughout the application process.
Alumnae: Alumnae transcript requests are processed through Parchment, our official transcript service.
- Create or log in to your Parchment account using the link above.
- Official Transcripts are sent directly to the receiving institution or agency.
- Cost is $5.25 per transcript plus a handling fee.
- Allow up to five business days for processing. You will receive an email notification when your request is complete.

Students interested in enrolling in community college courses or taking supplemental courses outside of Carondelet will need to complete a Special Admissions or Dual Enrollment form. As the designated school official, the Registrar is responsible for reviewing and signing the form for approval. Please keep in mind the following:
- Allow at least 48 hours for review. Forms submitted without adequate lead time may experience delays in approval.
- Submit to the correct email. The Special Admissions Form must be sent to registrar@carondeleths.org. Please do not send forms to narias@carondeleths.org, as submissions sent there will not be processed.
The Special Admissions or Dual Enrollment form is provided by the institution where you plan to take your courses. Please contact them directly for instructions on how to obtain and submit their form to us.
Will my external course appear on my Carondelet transcript?
Carondelet allows approved external courses to be listed on the official Carondelet transcript as supplementary coursework—so colleges can see your additional academic efforts without needing to request a separate transcript. However, please note that external courses will not be factored into your Carondelet GPA, count toward graduation requirements, serve as a prerequisite for a higher-level Carondelet course, or qualify for honors consideration.
For a course to be added to your official Carondelet transcript, it must meet the following criteria:
- Community college courses must be UC and/or CSU approved and transferable.
- High school courses must be UC approved and from a diploma-granting institution.
- An official transcript from the external institution must be sent directly to the Carondelet Registrar. Copies submitted by students or families are not considered official and will not be accepted.

Please note: courses will appear on your transcript exactly as received from the institution, including your final grade.
What if my course is not UC/CSU approved?
You may still include the course on your college applications and send the transcript directly to the institutions where you are applying—it just won’t be listed on your official Carondelet transcript.

All third-party enrollment verification requests must be submitted through Parchment.
Please note that original forms must be brought in to the Registrar’s Office in person for a signature, and families are responsible for delivering the completed forms to the Social Security office themselves.
To get your forms signed, you have two options:
- Schedule an appointment: Use Nattiel’s appointment link to book a time.
- Drop off in person: Bring your original documents to the Main Office during business hours.
The Original Form SSA-1372 BK must be included. Incomplete forms missing the school certification pages (pages 4 and 5) will not be certified.

Students who are legally eligible to work must complete a work permit before beginning employment. Please allow a minimum of 24 hours for processing—plan accordingly.
- Download and complete the Work Permit Form. A parent/guardian must sign the form.
- Submit your work permit to Lianna Adauto in the Main Office.
- Lianna Adauto will let you know when the work permit is ready. The student must then come to the Main Office to sign the official work permit in person, no exceptions. Parents or legal guardians cannot sign the permit on behalf of the student.
During the summer months, completed work permits can be picked up at the Main Office Monday through Friday, 8:00 AM to 2:00 PM.
Please note: work permits will no longer be issued online due to student compliance issues.

To update your demographic information (address, phone number, names, etc.), email registrar@carondeleths.org. Please make sure to send your request from an email account associated with the student.

- Name Changes: Legal documentation from the state is required for any permanent name changes. While Carondelet allows students to use preferred names or nicknames, official changes to the permanent record require legal verification.
- Emergency Contacts: To update emergency contact information, email registrar@carondeleths.org.
Updates to educational documents (IEPs, 504 plans, or legal documents) should be delivered in person to the Registrar at the Main Office.
For external schools and agencies: Requests for cumulative files or incoming records can be sent to registrar@carondeleths.org. Include the student’s full legal name and date of birth. If mailing physical records, send to:

The Office of the Registrar manages all College Board and NWEA official standardized testing, including the PSAT/NMSQT, SAT, Advanced Placement (AP) and MAP exams. For more information about these tests, please visit the College Board website and the NWEA website.

The Family Educational Rights and Privacy Act (FERPA) is a federal law that protects the privacy of student education records. In accordance with FERPA, Carondelet High School affords students and their parents or guardians the following rights:
- Right to Review: Parents and eligible students have the right to inspect and review the student’s education records.
- Right to Request Amendment: Parents and eligible students may request that the school correct records believed to be inaccurate or misleading.
- Right to Consent to Disclosure: Carondelet will not disclose personally identifiable information from a student’s education records without written consent, except as permitted by law (e.g., in response to a court order or in connection with a health or safety emergency).
An “eligible student” is a student who has reached 18 years of age and has graduated from Carondelet High School. At that point, rights under FERPA transfer from the parent to the student.
