Athletics Registration

There are several mandatory, annual actions that families and student-athletes must complete each year in order to participate in or volunteer for Athletics.

Upcoming Tryouts

To try out for an upcoming season of sport you must have completed your registration on All athletes must have a current copy of their physical to upload for registration. Physicals must be good for the entire school year. No paper copies will be accepted.

Students must complete their current “season of sport” before trying out for the next season of sport. Per NCS bylaws The season of a sport for any individual student is that period of time that elapses between the student’s first participation in an interscholastic contest and the student’s final participation in a contest for that particular sport in that season.

Get a Physical

  • Print and bring this Physical form to your doctor’s office. The form must be completed by a medical doctor (MD) or nurse practitioner (NP). Students need to communicate that the physical is for clearance for athletics participation.
  • It is strongly suggested that the physical exam be completed between June 10th and July 15th to comply with CIF regulations for athletics eligibility. Athletics physical forms are valid for one calendar year. If you are unable to complete your physical during these dates, make sure your physical is valid through your season of sport.
  • Make a copy of the completed form. All student-athletes will need to upload their own completed physical form. Carondelet staff cannot upload paperwork that has been previously submitted to the Enrollment Dept.

Note on Kaiser: On occasion, Kaiser physicians may not complete the school-approved physical form. Our recommendation is that students bring in the physical form. If the doctor will not complete the Carondelet form, the After Visit Summary needs to show that the student is cleared for unlimited participation in athletics.

Register on Athletic Clearance

Complete your student’s registration at Athletics registration for the 2022-23 school year will open on July 1.

All athletes trying out for Spirit Cheer and Dance must complete the Cheer/Dance Waiver and bring it with them to the first day of tryouts.

Academic Eligibility

Per the Student Handbook, athletes must maintain a minimum 2.00 GPA during their season of sport to maintain eligibility. If the Athlete fails to maintain a 2.00 GPA at each grading period or if she receives an “F” in any course at the quarter or semester, she will go on academic probation as determined by the Athletic Director, the Director of Academic and Learning Support, and the Vice Principal of Academics. She may remain on her team and participate in all practices, but participation in contests may be put in jeopardy until such time as cleared by the Director of Academic and Learning Support.

1. LiveScan Fingerprints

All adults who work with and volunteer at the school must complete fingerprint scanning. The Safe Environment Program requires that any volunteer having contact with Carondelet students have a fingerprint clearance – this serves as a protection for both you and students. Carondelet uses LiveScan.

Fill out the LiveScan request and take it to a location that offers LiveScan services. The location nearest to campus is The Postal Annex at Oak Grove and Treat. You will be responsible for the cost of the fingerprinting. Clearance will be submitted electronically to Carondelet.

If you have already completed fingerprinting through the Oakland Diocese, you still must complete this step for Carondelet.

2. TB Risk Assessment or Test

The California Health and Safety Code (§121545) requires all school volunteers to be examined and found free of communicable tuberculosis once every 4 years. Carondelet is required to keep an up-to-date certificate on file for each volunteer.

Please have the Adult Tuberculosis (TB) Risk Assessment Questionnaire certified by your healthcare provider or provide us with a copy of a current negative TB test. TB test results are good for four years from the test date.

3. Mandatory Training

Volunteers are required to take an online child abuse awareness training course, “Protecting God’s Children” in compliance with the Safe Environment Program at the Diocese of Oakland. If you have taken this training within the last three years, you can use that certificate for volunteering at Carondelet. If not, you can access the online training:

  1. Go to
  2. Click “First-Time Registrant”
  3. Click “Begin the registration process”
  4. Select “Oakland Diocese”

Once you have created a user ID you will be asked to select a school or parish. Please select “Carondelet High School” from the dropdown menu. When asked to the session you wish to attend, scroll down and select “Protecting God’s Children for Adults online training session.” You can self-register and take the training, which lasts about 90 minutes.

Keep a digital or printed version of the certification of completion to send to the Volunteer Coordinator.

4. License and Insurance

All adults driving students on behalf of Carondelet must submit a copy of their valid Driver’s License and Automobile Insurance Declaration page showing a minimum of $100,000/$300,000 liability coverage. Valid until insurance or drivers license expires. These copies will need to be resubmitted upon expiration.
